The EU (the organisation that cannot sign off its accounts) services directive has placed a requirement for us to make our application forms available electronically. The application process consists of a multipart application form. This form is quite long and you may not have all the information to hand.  The form has a facility to pause and you can open the form and resubmit the details later if you are missing some information. In order to use the re-submission facility you will need to make sure you are logged in with your username and password.

Access to the application forms is for registered users only. This is available after you have registered and downloaded a copy of the software. Please go to the software registration page and enter your details. You will then receive an e-mail with your username and password which will enable you to login to our website and access the sap accreditation forms.

The following list of documents are applicable to the sap assessor accreditation scheme.  All applicants must complete forms part 1 and part 2 plus the validation form. If you are a member of an approved professional body and are applying via the APEL route you must complete the APEL form.

The final part of the application process is to download & sign the declaration form. This needs to be scanned after signing and uploaded with your other documents. You can download a copy of the declaration form here.
